Ta-Nisha Emmanuel, DNP, FNP-C 
Ta-Nisha Emmanuel is DNP-Prepared Family Nurse Practitioner from Alabama and served in the military as an Army Combat Medic. While obtaining her degrees from Troy University, she worked in a variety of nursing fields with her current position in Home Health and Hospice. Her experience serving vulnerable populations has fostered a deep commitment to community health. Ta-Nisha’s interests revolve around serving these communities and creating policies and procedures that improve the continuity of care for patients and decrease hospital readmission rates. Progressing through life she continues to keep in mind, “Just because you stumble does not mean you must fall.”

Purpose
The purpose of this project was to examine the effect of the Patient Empowerment Program on hospital readmission rates and other clinical outcomes of geriatric patients with a primary diagnosis of heart failure in a home health setting. This presentation directly relates to the conference theme and objectives by offering to disseminate information on guidelines, quality improvement strategies, and education materials relevant to showing the DNP as a leader skilled in influencing policy in a specialized health care setting.
